just three easy steps to begin training with Special Olympics

1

Contact a local program near you to join a team or start your own.
Please visit our Local Programs page to determine the sport(s) of interest.  Once you have chosen a sport(s), please begin contacting teams practicing near you for information regarding joining their team.  This information is available on the Main page, Local Programs page, or by contacting Julie Smith, West Section Office Manager

If you don’t see a sport or team that you are interested in, please email , West Section Director to find out how to start a training program.

2

Complete the Athlete Application for Participation.
Every athlete must have a completed Special Olympics Massachusetts Athlete Application for Participation on file in the West Section office. This is simply a medical release form filled out by a parent, guardian or agency staff person, and the athlete’s primary care physician.

This medical form is valid for three years from the date of the last physical exam.  Once this form expires, a new form must be submitted to continue participation.

3

Send the completed, signed application to the West Section Office.
Once this form is completed and signed by the physician, please retain a copy for your records and send a copy to:

Special Olympics Massachusetts
West Section Office
Athlete Application
898 Prospect Street
Chicopee, MA 01020

or fax/email a copy to Julie Smith, West Section Office Manager

You must bring a copy of your completed and signed medical form with you to your first practice for your coach.

If you need further assistance, please email or call Julie Smith, West Section Office Manager.
